In Voices In My Head, i giocatori entrano in aula e nella mente di un uomo processato per aver rapinato una banca. Un giocatore assume il ruolo del pubblico ministero che cerca di condannare l'imputato, Guy, e mandarlo in prigione; tutti gli altri assumono aspetti della personalità di Guy, come l'Onestà o l'Egoismo, e cercano di influenzare il processo. Per vincere, ogni giocatore deve raggiungere il suo obiettivo nascosto.

Ogni turno, il pubblico ministero gioca una carta di processo per presentare prove, chiamare testimoni o interrogare Guy sul banco dei testimoni. I giocatori quindi cercano di controllare le azioni di Guy schierando marker di controllo su una plancia tridimensionale che rappresenta il suo cervello. Queste azioni influenzeranno la giuria in modi diversi e determineranno se Guy va libero o viene mandato in prigione.
